Hidden Vitamin B12 Deficiency: Many people assume that if their Vitamin B12 report is normal, then their health will also be fine. But Apollo Delhi surgeon Dr. Anshuman Kaushal, who is known as The Angry Doc on social media. He warned that normal reports do not always tell the truth. Many people continue to struggle with problems like fatigue, tingling feet, forgetfulness and irritability, while their reports seem absolutely normal.
Functional B12 Deficiency: When the report is fine, but there is deficiency in the body
Dr. Kaushal said in a video, “Have you seen people who are always tired, forgetful, depressed, but B12 comes normal in their report? This is functional B12 deficiency.” He explained that B12 is present in the blood, but the cells of the body are not able to use it. Giving an example, he said, “The B12 level looks perfect on paper, but in reality the salesman has nothing. Like having money in the bank account but no ATM card, looking rich, feeling poor.”
Why are normal tests not reliable?
According to Dr. Kaushal, most labs give reports only by measuring serum B12, whereas the real deficiency occurs at the level of cells. He said, “B12 and folate work like Batman-Robin. From DNA repair to making RBC and protecting neurons, both work together. If even one of these is deficient, the brain goes into ‘Gotham mode’. Stress, numb hands and legs, tingling all come for free.”
Which people are at greater risk?
Dr. Kaushal explains that some groups already fall in the risk zone. Among them, those taking metformin or acidity medicines, those on vegan diet and those who have undergone bariatric surgery are on the third. He says that if symptoms persist but the report is normal, then testing for MMA, homocysteine or active B12 is necessary. He said, “Many times tablets do not work and injections are required. The problem is not the vitamins, but the observation.” Functional deficiency means that your cells are not using B12 properly. Don’t go by the numbers, function matters. Save your neurons and don’t rely on B12 gummies for energy. This is biochemistry, not Bollywood.
Other doctors have also warned
Dr. Sudhir Kumar, a neurologist from Apollo, also told X that complaints of fatigue, forgetfulness and lethargy are common due to B12 deficiency, and normal reports do not always tell the whole story. He said that a large part of the B12 present in the blood remains attached to a protein which does not transport the vitamin to the cell. Due to this, B12 appears normal in the report, but the body does not get its benefit. This is the reason that despite the test being correct, fatigue and problems related to brain function may persist.
What is Vitamin B12?
According to the Cleveland Clinic, B12 is an essential nutrient that keeps nerves and red blood cells healthy and helps make DNA. The body does not make it itself, so it has to be taken from food such as meat, fish, eggs, dairy and fortified foods. Generally, adults need about 2.4 micrograms of B12 daily, while pregnant and breastfeeding women need more.
