Apart from this, you can also register yourself by visiting Ayushman App or the official portal of National Health Authority. There you have to check your name in the beneficiary list. After which your e-KYC is completed through Aadhaar OTP or fingerprint authentication and your digital card is generated.

Keep these documents ready with you
To take advantage of this entire scheme and get the card made without any hassle, you need some basic and important documents. The most important document is your Aadhar card, which is considered most important for your identity and biometric verification. Along with this, ration card or government family identity card is required to authenticate the names of all the family members.
Since this card is for farmers and economically weaker sections. Therefore, to prove your eligibility, you should also carry Kisan Samman Nidhi passbook or land record papers. Also keep an active mobile number with you so that you can get the OTP coming for verification immediately.
