Big news came amid the updates released in UP SIR, now it will be necessary to fill this form, these details will have to be given

Under the Intensive Revision Program-2026 issued in Uttar Pradesh, it is mandatory to fill the declaration form along with Form-6, in which the name and address of the applicant should be mentioned in the correct spelling, latest photograph and mobile number. The State Election Commission has given this information through a press release.

For age proof, birth certificate, Aadhaar, PAN, driving license, educational certificate or passport will be valid, whereas in case of absence of documents, affidavit as per prescribed age group or certificate issued by local body will be accepted, whereas for proof of residence, Aadhaar, one year old electricity-water-gas bill, bank passbook, passport, land records, rent deed or sale deed will have to be given and in their absence, on-site verification will be done.

It has been said in the press release that the details of self or family members will have to be filled in the declaration form from the voter list of the year 2003. If the details are not received, a notice will be issued, in response to which the prescribed records related to date of birth and place of birth will have to be submitted, while eligible citizens can submit Form-6 and declaration form online at voter.eci.gov.in or offline through BLO/ERO.

Chief Electoral Officer Navdeep Rinwa said that under the Special Intensive Revision Program-2026, the declaration form will be compulsorily filled along with Form-6 to be filled by the citizens. In Form-6, it is necessary to mention the name and correct address of the applicant in correct spelling, latest clear photograph and current mobile number.

What documents will be required for age certificate in UP SIR?

Birth certificate issued by competent authority
· Aadhar card
· PAN card
Driving License
· High school/intermediate certificate mentioning date of birth
Indian passport
· Any other document for proof of date of birth (if the above records are not available)

UP SIR: What to do if these documents are not there?

· In such cases, applicants in the age group of 18 to 21 years will have to appear before the Booth Level Officer/Assistant Electoral Registration Officer/Electoral Registration Officer along with an affidavit (Annexure-27) signed by their mother/father or guru (in case of third gender).
· If none of the above records are available and the parents are also not alive, then the applicant can submit the age certificate given by the Village Head/Municipal Corporation/Nagar Panchayat members.
· If the age of the applicant is 21 years or more, then he will present himself before the Booth Level Officer/Assistant Electoral Registration Officer/Electoral Registration Officer and submit only his declaration regarding age.

Which documents are required for residence in UP SIR?

· Water/electricity/gas connection bill (at least for one year) at that address
· Aadhar card
· Current pass book of National/Scheduled Bank/Post Office
Indian passport
· Land ownership records of Revenue Department, which also includes Kisan Bahi
· Registered rent lease deed (in case of tenant)
Registered sale deed (in case of own house)

In case any of the above records are not available, it will be necessary to get on-site verification done by the Electoral Registration Officer.

It is also mandatory to submit the declaration form along with Form-6 to be submitted under Special Intensive Revision-2026. In the declaration form, the applicant will have to fill in the details of himself or any one of his mother/father or grandfather/grandfather from the final voter list of the year 2003, along with assembly constituency number, part number and serial number. In case of correct mapping, no notice will be issued to the applicant. If the details of the previous Special Intensive Revision Electoral Roll-2003 given in the declaration form are not available or do not match the data base, then a notice will be issued to the applicant.

If notice comes in UP SIR, how will one have to respond?

1- Any identity card/pension payment order issued to a regular employee/pensioner of any Central Government/State Government/Public Sector Undertaking.
2- Any identity card/certificate/record issued in India by Government/Local Authorities/Banks/Post Office/LIC/Public Sector Undertakings before 01.07.1987.
3- Birth certificate issued by the competent authority.
4- Indian passport
5- Matriculation/academic certificate issued by recognized boards/universities
6- Permanent residence certificate issued by the competent state authority
7- Forest Rights Certificate
8- OBC/SC/ST or any caste certificate issued by the competent authority.
9- National Register of Citizens (wherever it is)
10- Family register prepared by state/local authorities.
11- Any land/house allotment certificate issued by the government.
12- Instructions issued by Commission’s letter No. 23/2025.ERS/Vol.II dated 09.09.2025 (Annexure II) will be applicable for Aadhaar.
13- Excerpt of voter list of Bihar SIR as per reference date of 01.07.2025.

It was told that if the applicant was born in India before 01.07.1987, then any one of the above 13 records will have to be provided for proof of date of birth/place of birth. Similarly, if the applicant was born in India between 01.07.1987 and 02.12.2004, then along with providing any one of the above 13 records for proof of date of birth/place of birth, the record of proof of date of birth/place of birth of the father or mother from the above list will also have to be provided.

If the applicant is born in India after 02.12.2004, then along with providing any one of the above 13 records for proof of his own date of birth/place of birth, he will have to provide the records of proof of date of birth/place of birth of both father and mother. If none of the parents is an Indian, then a copy of the valid passport and visa of that parent will have to be provided at the time of the applicant’s birth.

If the applicant was born outside India, then the Birth Registration Certificate issued by the Indian Mission abroad has to be attached and if Indian citizenship has been acquired by Registration/Naturalization, then the Citizenship Registration Certificate has to be attached.

Any eligible citizen can apply by visiting the Election Commission of India portal voter.eci.gov.in and filling online Form-6 (Declaration Form). You can fill the offline Form-6 along with the declaration form correctly and get it along with the necessary records to your BLO/Assistant Electoral Registration Officer/Electoral Registration Officer.
