Dr. Shailesh Jena, an endocrinologist at Shaheed Hospital in Raipur, says that this new guideline of WHO is a step taken in the right direction. According to him, “GLP-1 drugs have given better results than expected in the treatment of obesity. These drugs control appetite, improve metabolism and increase insulin sensitivity. In such a situation, they can prove to be a great help for patients on whom the effect of diet and workout is limited.” According to Dr. Shailesh Jena, obesity is not a simple problem of weight gain, but a metabolic disorder in the body, which needs long-term management.

Weight loss medicine: Sun Pharma’s new drug utriglutide, which is being developed to treat obesity and type-2 diabetes, has shown significant weight loss and metabolic improvements in post-menopausal women in early clinical studies. The results of the Phase 1a/2b trial of this drug were presented this week in the scientific session of the American Heart Association (AHA) held in New Orleans, USA. Utriglutide belongs to the same drug group as the world famous Monzaro and Vigovi. The specialty of these medicines is that they have become increasingly popular in India also. These medicines are GLP-1 receptor agonists, which work like liver hormones to reduce appetite.

What came out in the trial?

According to Sun Pharma, apart from weight loss, the drug showed improvement in several biomarkers related to systolic blood pressure, liver fat, and insulin sensitivity. Along with this, serum uric acid also reduced, which shows that the drug can affect many aspects of metabolic problems. Sun Pharma Chairman Dilip Shanghvi said, “Obesity and metabolic liver disease are on the rise, especially in post-menopausal women. The results of this initial study are encouraging and move us towards developing better treatments to combat these diseases.”

India’s anti-obesity drug market is currently worth Rs 3,000 to Rs 3,500 crore, which is expected to increase to Rs 25,000 crore by 2030. This was speculated to be due to the increasing problem of obesity and lifestyle diseases.

Study results give hope

This trial included post-menopausal women aged 52 to 69 years, who were suffering from obesity. After taking the drug for 14 weeks, those taking utriglutide lost an average of 8 percent weight and this continued until the 17th week. Those taking placebo lost only 2.1 percent and 1.2 percent weight, respectively.

What was seen till the 14th week?

• 76 percent women lost more than 5 percent weight.
• 25 percent women lost more than 10 percent weight.
• The average BMI initially was 43, which decreased to 39.7 to 39.8 by weeks 14 and 17.

The most important thing during this period was that there was a 28.6 percent reduction in liver fat, whereas only a 2.7 percent reduction was seen in the placebo group. These results show that this medicine can be helpful in problems like obesity as well as MASLD metabolic liver disease.

Side effect

If we talk about its side effects, they included loss of appetite, early fullness, nausea, indigestion and vomiting. Which are considered common among GLP-1 drugs. Dr. Rohit Loomba (UC San Diego), who was associated with the trial, said, “Utriglutide has shown significant improvement in liver fat and several metabolic parameters in women with obesity and MASLD, which is very encouraging.” If everything is approved after the trial, then this medicine can enter the market soon.

• The weight of the group taking fake medicine, which was barely 2 kg, decreased by 2 kg.
• The blood sugar level of patients taking the medicine also decreased rapidly.

Effective equal to injection

Experts say that this drug, which came in the form of a pill, is as effective as the GLP-1 class with injection. Experts believe that this medicine is safe and effective. At the same time, it can be a great option for patients who have trouble taking injections. Apart from reducing weight and sugar, this medicine also showed effect on many heart problems. The trial report revealed that the patient’s blood pressure, cholesterol and triglycerides have improved. Apart from this, the swelling in the body was reduced by about half i.e. 50%. The biggest thing came to the A1C level diabetes-free range of more than 70% of the participants who took more doses.

New generation medicines being used to reduce diabetes and obesity are nowadays worldwide discussion. They are known as ‘Blockbuster Weight Loss Drugs’. But do you know that these medicines can affect your eyes health? Recently, two new studies, published in Jama Network Open, have revealed this, after which a debate has started around the world whether it is really harmful to our eyes.

What medicines are in discussion?

Semaglutide: It is a GLP-1 hormone-like medicine, which controls blood sugar and reduces appetite. It is used for type-2 diabetes and weight loss.

Tirzepatide: It also acts like a gip hormone with GLP-1, which helps insulin release and weight control.

Which eye diseases are threatened?

These drugs have been linked to two serious eye problems in studies:

Diabetic retinopathy: Due to excess blood sugar for a long time, the vein of the retina deteriorates, which can lead to blindness.

Naion (non-arteritic anterior ischemic optic neuropathy): Suddenly, without pain, vision loss can occur due to reduced blood flow in the vein of the eye.

What came out in research?

In the first study, in patients with type-2 diabetes, GLP-1 drugs were compared to those taking other medicines. As a result, the risk of diabetic retinopathy and naion was found slightly higher than GLP-1 drugs. The good thing is that these drugs were less at risk of total blindness and the possibility of serious problems of retinopathy also declined.

In the second study, the most powerful GLP-1 drugs (semaglutide, tirzepatide) were found to be slightly increased cases of NAION and other optic nerve disorders. Both these studies have forced people to think once again.

Why was research needed?

If you talk about why it was needed, then there were indications of increasing the risk of diabetic retinopathy in the initial trial of Semaglutide about 10 years ago. After this, high-risk patients were kept out of further trial and monitoring was increased. Recently, two major research was done after the NAion case increased in a study. According to the European Medicine Agency, about 1 out of every 10,000 people who take semaglutide can have Naion. A similar study is also necessary for tirzepatide.

Should the medicine stop taking?

Experts say no. These drugs are a gamechanger in the management of diabetes and obesity. Also, their positive effects have been seen on heart, kidney and liver health.

What are the things to keep in mind?

• People who already have diabetic retinopathy must check the eyes before and after increasing the dose.
• Increase the dose gradually so that there is no sudden change in blood sugar.
• Keep better coordination between diabetes doctor and i-specialist.
• Eye safety should also be included in all future GLP-1 drug trials.
